Apparatus rooms, day rooms, office areas, bunk rooms, and storage areas are strictly off-limits to Lessees and/or attendees during events. All FD facilities are equipped with security cameras providing 24-hour surveillance.

I, the undersigned, by my signature below, do hereby acknowledge that I have read and accepted the BVFD Facilities Rental Agreement and the BVFD Liability Release and Indemnity Agreement and do hereby agree to this contractually binding agreement and do also agree to uphold all said rules and conditions of this agreement. My signature indicates my agreement to be fully responsible for these policies that I have reviewed and understood.

By signing this application, I acknowledge that I am making a request to reserve the facility indicated above. I understand that this is only a request to reserve and not an actual reservation. I understand that someone will contact me to follow up on this request and to finalize the rental should it be approved. understand this is a legal representation of my signature.
